MDS05-01 DIP relay. While rated at 5V, sample testing of these double-pole double throw relays indicates that they switch at voltages as low as 2.5VDC, and operational at higher DC voltages than 5VDC when used with appropriate dropping resistor so is an ideal small DPDT relay.
Midtex was founded in 1918 in Mankato, Minnesota with the invention of the synchronous motor timer.
